Hi everyone, I’m Dr. Chu Hsin-Cheng, a penis aesthetics expert specializing in male enlargement and thickening surgery.
Penis length is a topic many men care about.
People online claim everyone is 30 cm, but the average erect penis length in Taiwan is 12.6 cm—which is actually plenty.
Is there hope for a micropenis? If you think you’re already long enough, can you still get longer?
Today, let’s take a closer look.

Here’s the quick summary first; read on for more:
Budget-conscious → penile Botox
If you’re overweight → lose weight or get liposuction first, then consider other options
If you’re thin → suspensory ligament release + V-Y plasty lengthening

Concerned about scarring and appearance / limited budget → suspensory ligament release, with a small, discreet incision
If money is no object → acellular dermal matrix graft + V-Y plasty lengthening + suspensory ligament release for more size, girth, and length

Understanding penis anatomy

Let’s review the structure of the penis. It has three main parts:

Glans: This is the most sensitive area and is usually covered by the foreskin. After circumcision, it is often exposed, making it look larger.
Shaft: This is the part you can see, and it determines your “visible length.”
Root: This part is buried beneath the skin near the pubic bone, and many people don’t notice it. The length of the root that is visible is related to the amount of fat around your pubic bone: the more fat there is, the more deeply the root is buried, making the penis look shorter.

Today, we’ll focus on the “suspensory ligament.”
Suspensory ligament: Located at the base of the penis, it attaches the penis to the pelvic area. It consists of a superficial and a deep suspensory ligament.

How long does a penis need to be?

Generally, the G-spot is about 5–7 cm from the vaginal opening, and the prostate is about 3–4 cm from the anus.
An erect length of 7–11 cm is enough to provide sexual pleasure for a partner.
As for flaccid length, it’s a key factor in self-confidence.

Patients often ask me whether increasing “flaccid length” is meaningful.
I often ask them in return: Is your penis “flaccid” more often, or “erect” more often?

According to Korean doctors, in Korea, businesspeople often meet to discuss deals at saunas or bathhouses, where their penises are exposed.
In these settings, the size and length of a “flaccid” penis can affect the confidence conveyed during conversations and the other person’s perception, indirectly influencing whether a business deal succeeds.
Some people even get penis enlargement and lengthening surgery before entering the military, for fear of being bullied there.

What penis-lengthening surgeries are available?

Suspensory ligament release

This is the most common penis-lengthening surgery. It aims to increase visible length by cutting the suspensory ligament. Once these ligaments are cut, the penis, which was previously held back by them, can extend outward, increasing its length.
Advantages: Small incision, about 3–4 cm; increases “flaccid length” by 1–3 cm, with no change in erect length. Short recovery period.
Disadvantages: Cutting both the superficial and deep suspensory ligaments may reduce penile stability and change the angle of erection.

Cutting only the “superficial suspensory ligament” may also change the angle of erection, but the effect is minor and difficult to notice.

V-Y plasty lengthening

V-Y plasty lengthening is performed near the pubic bone to extend the skin where the penis meets the abdomen.

It can be performed together with suspensory ligament release.
Advantages: Increases “flaccid length” by 1–3 cm, with no change in erect length.
Disadvantages: Large incision (5–7 cm), noticeable scarring, and a long recovery period.

Acellular dermal matrix graft

This procedure uses an acellular dermal matrix graft to increase penis length, especially when flaccid.
Advantages: Small incision; increases “flaccid length” by 2–4 cm, with no change in erect length.
Disadvantages: Higher cost and a longer recovery period.

Pubic fat removal / liposuction

Some people have the base of their penis buried in a pad of fat. This fat can be surgically removed, or liposuction can be performed to expose the base of the penis and bring it back into view.
Advantages: Can effectively increase penis length; both “flaccid length” and “erect length” increase.
Disadvantages: Only suitable for people with excess fat.

Penile Botox injections

Botulinum toxin can relax the fascia of the penis, increasing its visible hang when flaccid. It is also much more affordable than surgery, making it a top choice for people on a budget.
Advantages:
●Noticeably increases flaccid length, making the penis look more prominent
●No surgery and virtually no wound; quick recovery
●Affordable
Disadvantages: The botulinum toxin breaks down after 4–6 months, so repeat injections are needed.

How long is the recovery period?

Recovery time varies by procedure. Generally:

Botulinum toxin injections leave only needle-sized marks and usually require 3–5 days of recovery.
Minor procedures such as liposuction and suspensory ligament release may have shorter recovery times, usually around 2–3 weeks.
More complex procedures such as V-Y plasty lengthening and acellular dermal matrix grafting usually require 1–2 months of recovery, along with regular checkups and aftercare.

How much does penis-lengthening surgery cost?

Costs vary widely by procedure and usually depend on factors such as the complexity of the surgery, the surgeon’s expertise, and the location. Here are some typical price ranges:

Penile Botox injections: NT$20,000–50,000, depending on the amount injected and the brand

Suspensory ligament release: NT$80,000–150,000

Acellular dermal matrix graft: NT$220,000–300,000, depending on the size and thickness.

Pubic fat removal / liposuction: NT$50,000–200,000, depending on the area treated and the brand of equipment.

V-Y plasty lengthening: NT$80,000–120,000

Penile implant: NT$250,000–500,000

Which procedure is right for me?

Everyone’s penis is different. Procedures for penile enhancement require an individual assessment and a detailed discussion of your needs.
The right procedure is different for everyone!

We recommend discussing your needs carefully with your doctor before surgery to achieve the results you want!
