What Should You Do About Premature Ejaculation? Can Surgery Really Help? A Look at Effective Medical Options

Hello everyone, I’m Dr. Chu Hsin-Cheng, the GG (penis) aesthetics expert, specializing in male enlargement and girth-enhancement surgery.

Today, let’s talk about a subject most men “don’t want to admit to, but have secretly looked up”: premature ejaculation.
There are quite a few posts on Dcard’s Relationships/Feelings board from people complaining about their own or their partner’s premature ejaculation.

Some people say, “I’m still young. This shouldn’t happen, right?” But premature ejaculation isn’t actually about age; it’s about “control.” The point isn’t how fast you are, but whether you can control it.

What is premature ejaculation? Is it an illness?

Not necessarily. Medically, “premature ejaculation” is defined by the following features:

  • Lifelong premature ejaculation: ejaculation almost always occurs before penetration or within about “1 minute” after penetration
  • Acquired premature ejaculation: ejaculation almost always occurs before penetration or within about “3 minutes” after penetration
  • Always or almost always unable to delay ejaculation
  • Causes negative psychological and emotional effects, such as distress, worry, frustration, and/or avoiding sex

An occasional “quick finish” doesn’t mean you have premature ejaculation. Being nervous at the beginning of a relationship or not having had sex in a long time can also be normal. But if you finish quickly every time, can’t control it, and your partner isn’t satisfied, it may be time to address the issue.

Premature ejaculation is also often accompanied by difficulty maintaining an erection. These two types of sexual dysfunction can affect each other and should be treated together!

Why does premature ejaculation happen?

There are many causes of premature ejaculation. We’ll divide them into two simple categories:[[ [DESIGNHU_TOKEN_000012]]]
※ Psychological (anxiety/depression)
●Too nervous or afraid of performing poorly
●Lack of experience; haven’t yet found the rhythm
●Too focused on the partner’s reactions, which can actually lead to losing control faster

※ Physical (overly sensitive)
●Overly sensitive glans
●Overly sensitive frenulum
●Tight foreskin; the glans has never been exposed
●Hormonal or neurotransmission abnormalities

So how can it be improved? Keep reading to find the treatment that’s right for you! We’ll introduce options ranging from conservative and behavioral treatments to surgery.

Conservative Treatment: Improve Premature Ejaculation Without Surgery

●Oral medication to delay ejaculation (Priligy) → acts on the brain’s central nervous system to increase ejaculation time by about threefold
-Pros: Few side effects; works quickly and well
-Cons: Works only when you take it; it’s a prescription medication that you pay for out of pocket and must be prescribed by a doctor

● Numbing spray/anesthetic cream → apply to the glans; after it’s absorbed through the skin, wash off any excess anesthetic to effectively reduce sensitivity
-Pros: Few side effects; works quickly and well; dosage can be adjusted
-Cons: Works only when applied; your partner may experience a numbing sensation

●Behavioral training → progressive training to reduce sensitivity (for example: the squeeze technique/Kegel exercises/massage)
-Pros: Few side effects; gradual approach
-Cons: Results vary; it can take several weeks of training to see results

For example, Taiwanese sex therapist Tong Songzhen invented the “人” (Chinese character for “person”)-shaped massage technique, which you can try at home. Remember to apply lubricant to prevent friction-related injuries to your penis.

Surgical Treatment: Addressing the Root Cause of Structural Sensitivity

If you clearly feel that you’re “really too sensitive,” or you’ve tried the methods above without success, you can consider the following surgical treatments:[[ [DESIGNHU_TOKEN_000045]]]
1. Circumcision
●Long foreskin / glans rarely exposed to the outside → circumcision may reduce sensitivity
●Improves odor and hygiene issues
●Improves appearance
●Recovery takes about 4 weeks

2. Frenulum removal
●A “frenulum” that’s too short can cause uncomfortable pulling
●After treatment, thrusting feels smoother, without pulling or stinging pain
●Reduces sensitivity
●Quick recovery, about 1–2 weeks

3. Dorsal nerve neurectomy
●Some people are born with more nerves and greater sensitivity;identify the dorsal penile nerves and selectively remove some of them
●Preserves sexual desire and the ability to ejaculate
●Longer operation time
●Recovery takes about 4–6 weeks

4. Glans augmentation
●Inject a small amount of hyaluronic acid beneath the skin of the glans to reduce sensitivity while making the glans fuller
●Inject allogeneic dermal matrix powder into the corpus spongiosum of the glans
●Minimally invasive; also makes the glans look fuller and improves its appearance
●Quick recovery, about 1–2 weeks; injections can be repeated

※ The procedures above can be combined or performed individually

With so many options, how should I choose?

The simplest and least expensive option: buy a bottle of lubricant and try behavioral therapy at home. If it doesn’t work well, see a urologist and try medication. If you’re still not satisfied after trying these methods, you can discuss surgical treatment with your doctor.

Conclusion

Premature ejaculation isn’t a flaw; it’s a condition that can be improved.
It’s not that you “can’t do it”—you just “haven’t learned to control it yet.” When you’re willing to face the issue, there are actually many ways to improve it, and they can work very well.
Many people find that after addressing it, they not only feel more confident in their sex lives, but also enjoy a better and more stable relationship with their partner.
